Official bankruptcy form changes effective December 1, 2015

Most official bankruptcy forms are scheduled to be replaced with substantially revised, reformatted and renumbered versions effective December 1, 2015. The new forms introduce different versions of petition forms for individual debtors and non-individual debtors.

Attorneys and filers should prepare for this major change in bankruptcy forms by:

  1. Checking our website for updated information www.lamb.uscourts.gov.
  2. Contacting your bankruptcy software vendor to insure that the vendor plans to provide a timely and proper update to the software to meet the new forms requirements. We have been informed that the software company EZ Filing will no longer be providing services after the implementation of the new forms.
  3. Reviewing new data and/or formatting requirements that have been incorporated into many of the official bankruptcy forms to make sure your internal office procedures are properly adjusted to gather required information during client interviews.
  4. Reviewing the two distinct versions of petition forms: one for Individual debtors and one for Non-individual debtors
  5. Taking note that current Exhibits B,C and D to the petition will be eliminated and the information will be incorporated into the petition. Exhibit A is now official Form 201A
  6. Reviewing the Combined Schedules: A/B and E/F.
  7. Making certain that entities/co-debtors on Schedules G and H are included on your official matrix.
  8. Reviewing new form 106J-2.
