Effective December 1, 2012, the following national forms will change:
- Official Form 7 Statement of Financial Affairs
- Official Form 10 Proof of Claim
- Official Form 21 Statement of Social Security Number or Taxpayer Identification Number
- Official Form 203A Notice to Individual Consumer Debtors
Please ensure that your bankruptcy software programs have been updated to include the new forms. You can also access the amended forms here:
